Inicialmente el conjunto se formó en 1978 bajo el nombre de "Sister", el cual se cambió a W.A.S.P. Su principal cabeza visible siempre ha sido su cantante, Blackie Lawless, nacido el 4 de septiembre de 1956. Su verdadero nombre es Steven Duren y vivía en Staten Island, Nueva York.

La estética de la banda y su puesta en escena eran abiertamente provocadoras haciendo referencia explícita al sexo y a la violencia, acorde con la letra de sus canciones y en los conciertos de sus inicios simulaban torturar a una mujer con el torso desnudo y una capucha negra en una especie de potro de tortura a la que simulaban cortar la cabeza y se desnudaba a una monja crucificada. También Blackie Lawless cortaba carne cruda con un hacha y se la arrojaba al público, cortaba un cerdo con una espada de samurai y bebía supuesta sangre de un craneo.
