An explosion at the Fukushima plant days after tsunami hit, caused by engineers releasing hydrogen gas that had built up after the cooling systems failed

The Mail is not getting it quite right here. Hydrogen explosion, indeed. That was what is known as an inadvertent criticality in reactor 3, the one fueled with MOX that contains plutonium.
