In designing a workout for the week, how do you choose volume?

Is it okay to pause/rest during a set? For instance, on a 12 rep set, is it okay to do 8 reps, rest for maybe 5-10 seconds, then finish the set?

What is that springy clip thing called that goes on the outside of the plates? I know many people don't use them, but I find them helpful when my lift fails and I drop the whole thing on the ground.
